The use of ice<\/span><span id=\"E632\">\u00a0or cryotherapy<\/span><span id=\"E633\">\u00a0in acute soft tissue injury is a well-known strategy to cope with the first problems of swelling and pain, despite a paucity of scientific data that support this strategy.<\/span><span id=\"E634\">\u00a0Throughout the years it became evident that\u00a0<\/span><span id=\"E635\">cryotherapy should not be an act on its own<\/span><span id=\"E636\">\u00a0but needs to be part of an integral approach.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p id=\"E637\"><span id=\"E638\">First, let us tackle\u00a0<\/span><span id=\"E639\">a general misconception<\/span><span id=\"E640\">:<\/span><\/p>\n<h2 id=\"E641\"><span id=\"E642\">Do you need to limit or eliminate inflammation?<\/span><\/h2>\n<p id=\"E643\"><span id=\"E644\">The answer is no, inflammation is necessary. The repair will not occur without inflammation, and even if you want to, you cannot eliminate inflammation; you can only minimize the signs of infl<\/span><span id=\"E645\">ammation l<\/span><span id=\"E646\">ike is shown in a study from Lee and colleagues (2005).<\/span><\/p>\n<p id=\"E647\"><span id=\"E648\">When a tissue is injured<\/span><span id=\"E649\">,<\/span><span id=\"E650\">\u00a0the\u00a0<\/span><span id=\"E651\">cardinal\u00a0<\/span><span id=\"E652\">signs Rubor (redness), Calor (heat), Edema (swelling), Dolor (pain) and Funca Laesa (functional loss) will occur to some degree, and the body responds with the inflammatory response.<\/span><\/p>\n<p id=\"E653\"><span id=\"E654\">T<\/span><span id=\"E655\">he healing phase<\/span><span id=\"E656\">s\u00a0<\/span><span id=\"E657\">associated are:<\/span><\/p>\n<ol>\n<li id=\"E658\" class=\"qowt-li-6_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E659\">the inflammatory phase (acute care),<\/span><\/li>\n<li id=\"E660\" class=\"qowt-li-6_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E661\">the proliferation phase (sub-acute care) and<\/span><\/li>\n<li id=\"E662\" class=\"qowt-li-6_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E663\">the maturation phase (post-acute care).<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p id=\"E664\"><span id=\"E665\">In each of these phases, the way local cryotherapy\u00a0<\/span><span id=\"E666\">should be\u00a0<\/span><span id=\"E667\">applied\u00a0<\/span><span id=\"E668\">is\u00a0<\/span><span id=\"E669\">different\u00a0<\/span><span id=\"E670\">and even finds a transition point, the point where no longer c<\/span><span id=\"E671\">ryotherapy<\/span><span id=\"E672\">\u00a0but<\/span><span id=\"E673\">\u00a0local<\/span><span id=\"E674\">\u00a0heating should be utilized. The complete process\u00a0<\/span><span id=\"E675\">needs to be driven by clinical decision making<\/span><span id=\"E676\">;<\/span><span id=\"E677\">\u00a0a proper physical examination, a patient interview and notice of the response to prior treatment.<\/span><\/p>\n<p id=\"E678\"><span id=\"E679\">The goal<\/span><span id=\"E680\">s<\/span><span id=\"E681\">\u00a0of cryotherapy during your rehabilitation\u00a0<\/span><span id=\"E682\">in the different phases\u00a0<\/span><span id=\"E683\">are<\/span><span id=\"E684\">:<\/span><\/p>\n<ul>\n<li id=\"E685\" class=\"qowt-li-2_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E686\">Facilitating rehab exercises<\/span><\/li>\n<li id=\"E687\" class=\"qowt-li-2_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E688\">Pain management<\/span><\/li>\n<li id=\"E689\" class=\"qowt-li-2_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E690\">Limit analgesic use<\/span><\/li>\n<li id=\"E691\" class=\"qowt-li-2_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E692\">L<\/span><span id=\"E693\">imit arthrogenic muscle inhibition<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span id=\"E695\">While to increase effectiveness, the appropriate cryotherapy duration should be determined by measuring the skinfold thickness at common cryotherapy treatment sites (Jutte et al. 2012).\u00a0<\/span><span id=\"E697\">This dependency on skinfold thickness is also illustrated in figure 1; a larger adipose tissue implies longer treatment durations to become the same tissue temperature drop.<\/span><\/p>\n<p><img decoding=\"async\" src=\"https:\/\/cdn2.hubspot.net\/hubfs\/2365968\/Screenshot%202018-06-13%20at%2009.57.29.png\" alt=\"Screenshot 2018-06-13 at 09.57.29\" width=\"312\" \/><span id=\"E715\">(F<\/span><span id=\"E716\">igure 1;\u00a0<\/span><span id=\"E717\">derived from\u00a0<\/span><span id=\"E718\">Otte\u00a0<\/span><span id=\"E719\">et al.\u00a0<\/span><span id=\"E720\">2002<\/span><span id=\"E721\">).<\/span><\/p>\n<p>&nbsp;<\/p>\n<p id=\"E722\"><span id=\"E723\">In time the c<\/span><span id=\"E724\">ryotherapy strategy\u00a0<\/span><span id=\"E725\">(ICE), transferred\u00a0<\/span><span id=\"E726\">in<\/span><span id=\"E729\">to RICE (rest and cooling)<\/span><span id=\"E730\">,<\/span><span id=\"E731\">\u00a0into PRICE (Protection<\/span><span id=\"E732\">, Rest and cooling).<\/span><span id=\"E733\">\u00a0Bleakley and colleagues (2012) after<\/span><span id=\"E734\">\u00a0that,<\/span><span id=\"E735\">\u00a0suggested POLICE<\/span><span id=\"E736\">, a<\/span><span id=\"E737\">\u00a0new acronym, which represents protection, optimal loading, ice compression, and elevation.<\/span><span id=\"E738\">\u00a0<\/span><span id=\"E739\">In this approach, o<\/span><span id=\"E740\">ptimal loading\u00a0<\/span><span id=\"E741\">is integrated<\/span><span id=\"E742\">,<\/span><span id=\"E743\">\u00a0as new insights consistently provide evidence that mechanical loading upregulates key proteins associated with soft tissue healing.<\/span><\/p>\n<p id=\"E744\"><span id=\"E745\">Besides new scientific insights, new technolog<\/span><span id=\"E746\">ies are<\/span><span id=\"E747\">\u00a0<\/span><span id=\"E748\">developed\u00a0<\/span><span id=\"E749\">and available on the market. The<\/span><span id=\"E750\">s<\/span><span id=\"E751\">e<\/span><span id=\"E752\">\u00a0technolog<\/span><span id=\"E753\">ies<\/span><span id=\"E754\">\u00a0<\/span><span id=\"E755\">make it possible to break with\u00a0<\/span><span id=\"E756\">\u201c<\/span><span id=\"E757\">the 20 minutes habit<\/span><span id=\"E758\">\u201d<\/span><span id=\"E759\">\u00a0<\/span><span id=\"E760\">thereby being able\u00a0<\/span><span id=\"E761\">to cool\u00a0<\/span><span id=\"E762\">down\u00a0<\/span><span id=\"E763\">intra-articular temperature<\/span><span id=\"E764\">s<\/span><span id=\"E765\">. While other technolog<\/span><span id=\"E766\">ies, like cryogenically cooled air,\u00a0<\/span><span id=\"E767\">can decreas<\/span><span id=\"E768\">e superficial\u00a0<\/span><span id=\"E769\">tissue temperature more rapidly by the use of much colder temperatures<\/span><span id=\"E770\">\u00a0(up to\u00a0<\/span><span id=\"E771\">\u2013<\/span><span id=\"E772\">140<\/span><span id=\"E773\">\u00b0<\/span><span id=\"E774\">C or -220<\/span><span id=\"E775\">\u00b0<\/span><span id=\"E776\">F)<\/span><span id=\"E777\">.<\/span><\/p>\n<p id=\"E778\"><span id=\"E779\">The<\/span><span id=\"E780\">se<\/span><span id=\"E781\">\u00a0new technologies will help to make the use of cryotherapy more effective.\u00a0<\/span><span id=\"E782\">It should be\u00a0<\/span><span id=\"E783\">remembered though<\/span><span id=\"E784\">\u00a0(a<\/span><span id=\"E785\">ccording to Bleakley et al.,\u00a0<\/span><span id=\"E786\">200<\/span><span id=\"E787\">4<\/span><span id=\"E788\">) that<\/span><span id=\"E789\">:<\/span><\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li id=\"E790\" class=\"qowt-li-0_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E791\">An optimal tissue temperature reduction of 10<\/span><span id=\"E792\">\u00b0<\/span><span id=\"E793\">C to 15<\/span><span id=\"E794\">\u00b0<\/span><span id=\"E795\">C is necessary to maximize the therapeutic effect.<\/span><\/li>\n<li id=\"E796\" class=\"qowt-li-0_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E797\">The skin temperatures should drop to below\u00a0<\/span><span id=\"E798\">13,<\/span><span id=\"E799\">6<\/span><span id=\"E800\">\u00b0<\/span><span id=\"E801\">C to achieve local analgesia and<\/span><\/li>\n<li id=\"E802\" class=\"qowt-li-0_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E803\">A tissue temperature between 10<\/span><span id=\"E804\">\u00b0<\/span><span id=\"E805\">C and 15<\/span><span id=\"E806\">\u00b0<\/span><span id=\"E807\">C may be required to maximally lower metabolism.<\/span><\/li>\n<\/ul>\n<p>Based on all this, what\u2019s new in the use of local cryotherapy?<\/p>\n<p>&nbsp;<\/p>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<ul>\n<li id=\"E810\" class=\"qowt-li-1_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E811\">ICE-&gt; RICE-&gt;PRICE-&gt; P<\/span><span id=\"E812\">OLICE, position\u00a0<\/span><span id=\"E813\">local\u00a0<\/span><span id=\"E814\">cryotherapy in an integral approach that also focusses on optimal loading<\/span><span id=\"E815\">.<\/span><\/li>\n<li id=\"E816\" class=\"qowt-li-1_0 qowt-list qowt-stl-Lijstalinea\"><span id=\"E817\">In the acute care phase,\u00a0<\/span><span id=\"E818\">the\u00a0<\/span><span id=\"E819\">focus should be on applying local cryotherapy which shifts to optimal loading in the proliferation phase and maturation phase where local cryotherapy\u00a0<\/span><span id=\"E820\">only\u00a0<\/span><span id=\"E821\">assists.<\/span><\/li>\n<li class=\"qowt-li-1_0 qowt-list qowt-stl-Lijstalinea\">Clinical decision making should determine the transition point from applying local cryotherapy to local heating<span id=\"E825\">.<\/span><\/li>\n<li class=\"qowt-li-1_0 qowt-list qowt-stl-Lijstalinea\">New technology<span id=\"E828\">, using very low temperatures,<\/span><span id=\"E829\">\u00a0<\/span><span id=\"E830\">m<\/span><span id=\"E831\">ake<\/span><span id=\"E832\">\u00a0l<\/span><span id=\"E833\">ocal cryotherapy more efficient.<\/span><\/li>\n<li class=\"qowt-li-1_0 qowt-list qowt-stl-Lijstalinea\">Local cryotherapy should be customized taking body composition, gender, and fitness into account<span id=\"E836\">.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span id=\"E838\">In summary,\u00a0<\/span><span id=\"E839\">the duration, intensity, and frequency of local cryotherapy should be determined by t<\/span><span id=\"E840\">he repair phase, the injured tissue, depth of injury, personal characteristics and cooling modal<\/span><span id=\"E841\">ity.<\/span><span id=\"E842\">\u00a0The new technologies for local cryotherapy can be applied in other domains than sports or rehabilitation as well as<\/span><span id=\"E843\">\u00a0for<\/span><span id=\"E844\">\u00a0vitally or beauty needs.
